Awards of Excellence

Congratulations to this year's winners!

The Society of Hospital Medicine created its Awards of Excellence Program to honor our members whose contributions
to the Hospital Medicine movement deserve acknowledgment and respect.

In honor of their achievements, recipients of each Award of Excellence receive an all-expense paid trip to SHM's
Annual Meeting!

Recipients also benefit from:

• Recognition on stage, in front of friends, family, and colleagues, at Hospital Medicine 2012
• Recognition in the The Hospitalist, SHM's monthly news magazine, and on www.hospitalmedicine.org
• Recognition in press releases to local/state outlets

Eligibility for Awards

Any SHM member who fits the following definition is eligible for nomination for an award:

A physician whose primary focus is the general medical care of hospitalized patients. The physician's activities include patient care, teaching, research, and leadership related to hospital care.

Current SHM Board members and Officers as well as SHM Awards Committee members are not eligible.

Nomination Requirements and Information (nominees may be self-nominated)

All submissions require the following:

• Nominee must be an SHM member
• Completed Nomination Form, addressing the specific criteria that are the basis for evaluation of the nominee
  being considered
• Nominee's Curriculum Vitae submitted with the Nomination Form (it's requested that a shortened CV be submitted that is no more than 3 pages in length)
• Two letters of recommendation in support of nominee (if more than two letters are submitted, nominee will be contacted to specify which should be considered; each letter should indicate if confidentiality of content is waived)

Please be sure to have one person collect and submit the Nomination Form, CV, and two letters of recommendation on behalf of the nominee to awards@hospitalmedicine.org. Failure to submit all the required information together will result in an incomplete nomination.
